Every surrogacy journey begins with a medical process that’s filled with precision, care, and hope. IVF and embryo transfer are the foundation of this journey, and understanding how it works can ease anxieties and build trust for both intended parents and surrogates.
What Is IVF?
In vitro fertilization (IVF) is the process of combining an egg and sperm outside the body in a lab to create an embryo. This embryo is later transferred into the surrogate’s uterus.
In surrogacy, the eggs may come from the intended mother or an egg donor, while the sperm usually comes from the intended father. Once the embryos are created, the healthiest one is selected for transfer.
Preparing for Embryo Transfer
Before the transfer, the surrogate undergoes medical screenings to ensure she’s healthy and her uterine lining is ready. Hormonal medications—like estrogen and progesterone—are prescribed to prepare her body to receive the embryo.
Throughout the preparation phase, there are frequent ultrasounds and blood tests to monitor hormone levels and lining thickness. The goal is to ensure everything is perfectly timed for implantation.
What Happens on Transfer Day?
Embryo transfer is a simple, outpatient procedure that takes about 10 to 15 minutes. No anesthesia is needed. Using a thin catheter, the fertility doctor gently places the embryo into the uterus.
After the transfer, the “two-week wait” begins. It’s an emotional time as everyone hopes for a positive pregnancy test. During this period, the surrogate is advised to rest, stay hydrated, and follow all medical instructions carefully.

If It Doesn't Work the First Time
Sometimes embryo transfers don’t lead to pregnancy. That’s okay—it’s more common than people think. If this happens, the medical team will review everything and may recommend trying again with adjustments.
